To qualify for the boutique, participants must have a referral form completed by a guidance counselor, teacher, church leader or social service representative. All forms should be faxed back no later than April 14, 2008.

All forms will be reviewed and referred candidates will receive notification in mid April. Our goal is to provide an opportunity to all qualified participants. Participants are expected to RSVP.
